10-15 Minute Initial Phone Consultation…. No fee

25 Minute Medication Management Appointment ... $250

45 Minute Medication Management and/or Psychotherapy Appointment ... $350

60 Minute Intake Appointment ... $475

APPOINTMENTS

​

Appointments are available via Zoom for patients located anywhere in California, or in-office in Sacramento. If you would like to schedule an appointment, please call (916) 314-2994, or complete the form below. I will reach out to you, and we can determine if my practice is the right fit for you through a brief phone consultation. If we decide to move forward, we will schedule an appointment, and I will send you a link to my patient portal (Charm). You will then be prompted to fill out my intake forms.

PAYMENT

​

I do not accept insurance, and am considered an out-of-network provider. Payment is due in full at the time of your appointment, and may be submitted by cash, check, any major credit card or your HSA card. Many insurance companies will provide partial or full reimbursement for services rendered by an out-of-network psychiatrist. If you would like, please reach out to your insurance company to ask about their reimbursement policies. Upon request, I am able to provide you with a superbill through the patient portal to submit to your insurance company for reimbursement.

CANCELLATION POLICY

Should you need to cancel or reschedule an appointment, please do so with 48 business hours notice. You will be responsible for the full appointment fee if the appointment is not kept or not cancelled with appropriate notice.
